- Cyndi Berg, PsyD, University of Utah, invited the work group to participant in a 10-15 minute survey that her center is administering.
- Two aims of the study and survey link:
- Understand social context of CGM data sharing
- Use the results to inform changes to clinical care that incorporate support persons (SPs)

- Dr. Ross reviewed the Six Core Elements of Health Care Transition and how we will use this framework to guide this work group.
- Katrina Hoch PhD, MS, RD, CDCES and Kate Weaver, MD, University of Washington and Seattle Children’s Hospital, presented on the AHEAD Program, a collaboration between the University of Washington and Seattle Children’s Hospital to assist in transitioning to adult care.
- The pogrom is externally funded by Breakthrough T1D. and bridges two institutions (one pediatric and one adult) to provide uninterrupted care.
- Katrina shared details about the program model, multidisciplinary care, and the AHEAD Philosophy.
- The AHEAD program works off adult and pediatric standard care but ads additional touchpoints and screenings to improve transition readiness.
- Enrollment information and data were shared. The results are promising and the group requests Katrina returns and reports back in the future.
