Discussion centered on the Texas Children’s smart form and related workflows.

The Turbocharger package in Epic was reviewed.

Form Design Notes

Date fields are calendar-based to ensure standardized and accurate data entry.

“Known” vs. “N/A” status was considered confusing; recommendation to revise to “Known” vs. “Unknown.”

All fields are discrete data elements captured within Epic.
Questions & Discussion Points

Intended Population: The form is not intended for the entire Type 1 Diabetes population. It is specifically designed for a screening/staging program.

Form Functionality: The form serves as an ongoing touchpoint and can be updated whenever the patient is seen.

Recommendation to add “No family history” as an option under family member history instead of leaving it unselected.

For family history marked as “Other,” the group agreed to avoid collecting PHI and simply record “Other.”

Clinical Workflow: The form is designed for clinicians already performing T1D screening in high-performing centers. Detailed staging guidance should not be embedded in the form; staging decisions should remain clinician-driven and aligned with ADA standards.
Data & Staging
- Data will display similarly to a flowsheet, preserving historical entries rather than overriding them.
- Users will be able to view Stage 1, Stage 2, and Stage 3 dates.
- Stage 3 corresponds to the diagnosis date previously discussed for T1DX inclusion criteria.
- If a patient regresses, users can go back and override the original stage date.
- However, patients generally do not regress
Texas Children’s Registry & Dashboard
- Texas Children’s maintains a separate registry for screening patients.
- A data specification for this registry will be shared with the group.
TCH Diabetes Prevention Dashboard
- Provides a high-level overview of data collected from the smart form.
- Reports can also be generated from the dashboard.
System Updates
- Dr. DeSalvo can make updates to the TCH system.
- The version demonstrated during the meeting recently went live.
- Future changes would require organizations to re-adopt the Turbocharger package; updates will not occur automatically.
Template (James Dawson)
- Add a yes/no family history row above Row 6.
- Laura can rename the AABs.
- James will share his template with the group.
Open Questions / Follow-Up
- Clarify the time investment required for Turbocharger package installation (question for Kelly from Texas Children’s).
- Once installed, the package remains installed.
- Ensure the form is reviewed carefully during Tuesday’s call, particularly with the adult representative attending.
- The new screening and monitoring elements that will be incorporated into the spec will be added to a special project sheet and will not be released at the same time as data spec 040
