Gwendolyne Anyanate Jack, MD, MPH

Dr. (Anyanate) Gwendolyne Jack is an Assistant Professor of Clinical Medicine at Weill Cornell Medicine’s Division of Endocrinology, Diabetes, and Metabolism. She received her Bachelor of Arts degree from New York University. She earned her Masters of Public Health from Columbia 
University Mailman School of Public Health. She earned her Medical Doctorate at Sidney Kimmel Medical College in Philadelphia. She completed her internship and Internal Medicine Residency at Johns Hopkins Bayview in Baltimore. She completed her fellowship in Endocrinology, Diabetes, and Metabolism at Beth Israel Deaconess Medical Center/Joslin Diabetes Center at Harvard. She is board certified in Internal Medicine and Endocrinology, Diabetes, and Metabolism.
She is the Lead of the Ambulatory Diabetes Program at Weill Cornell Medicine Division of Endocrinology, Diabetes, and Metabolism, where she developed a clinical workflow for the optimizing the visualization of glucose data and connected diabetes technology devices. She currently is an Advisor of the American Diabetes Association (ADA) Diabetes Technology Interest Group. She has served as co-author on several publications and has been an invited speaker to several national presentations focused on integrating diabetes technology using a team-based approach to optimize clinical workflow and applying an equity lens to diabetes care. She is the Weill Cornell Community Health Champion, and spearheads several efforts to address food insecurity by developing the food insecurity screening and referral process and collaborating with community gardens. She has received several awards for Excellence in Mentorship, Excellence in Medical Education, Leadership in Community Engagement. Dr. Jack is passionate about empowering individuals with diabetes, community engagement, medical education, and enhancing the learner-patient-clinician-community partnership.
