Agenda:

  • Introductions- Ryan McDonough and Nirali Shah
  • T1D Exchange Updates- Emma, T1D Exchange
  • Q4 DSC meeting-last of the year -Emma, T1D Exchange
  • Creating a T1DX-QI Data Mapping Guideline- James, T1D Exchange
  • Improving care for T2D and nephropathy- Dr. Nirali Shah
  • Remote Monitoring Data- Dr. Ryan McDonough

Type 1 Diabetes Mapping Updates

  • 46 centers fully mapped
  • 6 in validation phase

Type 2 Diabetes Mapping Update

  • 5 centers in production
  • 3 centers still in the mapping process

T1DX-QI Scorecards

  • Currently in production and will be sent out by the end of September to mapped centers
  • In the future we will be sharing these annually

Creating a Data Mapping Guideline

  • 2-3 page guide for clinics to help them have an SOP so that if there are staffing changes, people know how to do essential things
  • What would new people have to know to do the next data pull or make a modification?
  • James shared draft document and will be adding in some plainer language and definitions and will share final draft

“FINER Care” Optimizing Care for Diabetic Nephropathy: A Quality Improvement Initiative with Finerenone Presentation

  • Overview of project and will be rolling out an OPA (Our Practice Advisory) for Finerenone in patients with type 2 diabetes among other inclusion and exclusion criteria
  • Will plan to share updates once the OPA is rolled out-possibly to expand collaborative wide
  • Share feedback with current BPA project in collaborative

Remote Monitoring Data

  • Possibly a new area to collect data-how do we collect this?
    • Collecting alerts in the background of how often devices are used/altered to
    • How frequently patients are opening their diabetes related apps
    • Does this lead to increased burden?
    • Challenges exist to this data collection and quantifying these definitions
  • How can we evaluate if this non-stop availability poses possible negative impact to the patient/parent wellness or patient glycemic control ?
  • Goal: to quantify the amount of time parents/patients spend observing their glucose data, and to evaluate if this non-stop availability poses possible negative impact to the patient/parent wellness or patient glycemic control
  • Possible starting point– quantitatively evaluate the impact of time spent in remote monitoring apps on parental wellness, patient self-efficacy, and glycemic outcomes in youth and adolescents with T1D
  • Additionally, would be interesting relating to camp as well and parental control/breaks from phone

Future meeting discussions: the use of AI scribe and capturing this unstructured data
