Bringing PWD perspectives to Collaborative Work:
- Transitions of Care
- Michigan Diploma Project
- Care by location
- ISPAD website (for connecting care outside of the US)
- Location changes not always necessary due to remote care
- Provider Registry- recommendation for clinicians: highlight areas of interest so that new patient know subject matter interest (devices/technology, transitions of care, etc) to match better
- Community Health Worker as a bridge between care
- Student Health – discussed Diabetes Link as a potential resource for college/clinic resources
- 1 Page Transition Preparation Guide highlighting key areas of importance (what to do in emergencies, problem solving around supplies, medications, prior authorizations, insurance, etc.)
- Diabetes Distress
- 5-6 diabetes distress scales are being tested by 15 practices in the Collaborative now.
- Diabetes clinicians recognize the importance of having conversations with patients about their distress/mental health in real time (without depending on a psych expert to lead later.)
- Will use time during November Learning Session to offer practical training for clinicians so that they can more effectively discuss diabetes distress with PWD
