Rationale

• T1D is increasing and worse outcomes for Hispanic and NH Blacks
• 7 sites participating in pilot site
• CGM Use:
  • Prescription to DME or Pharmacy
• Pump Use
  • Communication
  • Provider bias
• QI Tools are useful in increasing equitable CGM and Pump Use
• Parent/Patient participation is important in brainstorming
• Monthly team meetings useful for sharing
