Adult  Collaborative Call Meeting Minutes

January 24, 2023, 3:30-5:00pm

• Welcome & introductions
• Updates from the Collaborative
  • We currently have 54 participating clinics, 34 Pediatric & 20 Adult
  • Welcome Johns Hopkins and UC Davis
    • PI: Nestoras Mathioudakis, MD, MHS

  • November Learning Session Reimbursement
    • We will not accept reimbursement after 1/30/2023
      • Reimbursement form
    • 2023-2025 Reporting
      • Reminder: reporting for the 2023-2025 period, which began 1/1/2023.
      • Expectations: centers should report monthly data for the Jan 1-31, 2023 period by 3/1/2023.
      • Find new Reporting Measures
    • T2D Program
      • 2023-2024 Expansion: 3 additional sites (BMC, Grady, University of Pittsburgh)
      • 2023-2024 Demonstration: Co-authorship opportunities, contribute to limited T2D measures (5-6) in smart sheet.

  • Learning Objectives:
    • To describe strategies and identify tools and processes that facilitate screening for Social Determinants of Health
    • 8 question survey with prompts to indicate yes
    • Formal referral process was created to Social Work
      • Language and script was tailored to encourage patients to accept referrals
    • Clinical training for staff on screening and documenting
    • Expanded language to 6 additional most commonly spoken languages of the population
    • Next Steps: increase referrals for positive screening

    • Measuring data by: data mapping, Smartsheet, special project reporting
