- 2000 pts seen qtly
- Multiple campuses. Diabetes care takes place in outreach clinics, primarily.
- Started screening PAID-T, 14 item, validated in youth.
- Changed EMR. Previous used screeners where pdfs could be uploaded.
- Started in Feb. 2017. Linked to roll out of PHQ-9 at main campus
- Pilot March 2018
- EMR change 9/2020
- Started at just 2%. Rose to 80% screening. 22% endorsed elevated distress
- Built screeners in Epic.
- MAs manually send messages about telemed visits
- Positive screeners appear in red. Did not make into a BPA. Provider discussion.
- Can look at item level data for discussions.
- MyChart messages
- Aim to roll out everywhere
- Updated templates to auto assign
- Data from flowsheet, increased in March 2024- to close to 50%.
- Challenges: MA turnover. Trying to train to reinforce completing screeners before visit.
- Q: retraining is important. Have flowsheet and mandate use to sign off on the encounter. Now less likely to miss.
