Transitions of Care Adult Focus

What is our goal?
- Should we make a common goal, or is It hard because centers are all in different places
- Heterogeneity could be beneficial based on obstacles and challenges at each center
- Create an outcome metric and allow centers to determine process that makes sense for their center
Timeframe expectations
- Should we create a concrete standard on time in between transfer
- 6 months minimum
Receivership
- First initial adult visit tracked as an adult measure and then
- Retention in 6 months
- Retention in 1 year
Clinic Coordinator/Resources
- Adult practices are not receiving new patients for at least 6 months
- Depending on practice size, manual tracking could be an option (not as ideal) but if small could be less data points to follow up on
Big Idea: Adults will begin to think about creating a measure on Retention after receiving from the pediatric side
