Attendees: Emma Ospelt, Trevon Wright, Mary Pat Gallagher, Risa Wolf, Nes Mathioudakis, Rachel Hopkins, Jerusha Owusu-Barnie, Susan Thapa, Joseph Erardi, Ruth Weinstock, Justin Indyk, Manu Kamboj, Yasi Mohsenian, Asher Beckwitt, Faisal Malik, Juan, Laya Chadalawada, Sabeena, Josephine Kolawole
Today’s Meeting to Discuss:
o Trevon shared project overview, study timeline, project milestones, and Statement of Work updates.
o Participating centers are eligible to invoice for deliverable 3 and can invoice for deliverable 2 no later than May 2025.
o Update on the protocol paper under review in JMIR.
o Dr Mathioudakis shared a potential prototype for the BPA logic/ design.
o The group reviewed the current BPA design, logic, and criteria for the build within the context of Johns Hopkins.
o Demo the BPA with a patient in the Epic proof of concept environment.
o Dr Mathioudakis shared the timeline on BPA integration. Formal request for review in mid March. Move towards getting it into the production environment at Hopkins in late March/early April. Provider training and working with site clinicians and IT leaders to build into their context.
o The group discussed BPA timing and possible workflow interruption.
o The group discussed alternative triggers and other possible solutions.
o Josephine suggests using a different or additional trigger action, such as updating a flow sheet. Juan suggests using an open patient encounter with a pop-up lockout and then using either storyboard or general BPA section. The pop-up lockout can be set to happen once per user. A snooze button can be added to the alerts to defer action to a later point.
o Final Wording and Consensus for BPA Design:
o “Patient ready, deciding on pump” is suggested to highlight patient readiness and the support needed during the decision-making process. The final acknowledgement reasons are: Not T1D, will update problem list. Patient declined, not ready. Risk outweighs benefit. Cost/insurance. Pump ordered, not started. Patient ready, deciding on pump.
o The group aims for language that is clear and easily understood, even by those unfamiliar
with the process.
• Next Steps:
o T1DX-QI leadership team will coordinate meetings with Dr. Mathioudakis, Josephine, and their counterparts to share the logic behind the code and adapt it for their context with the participating centers.
o The goal is to start sharing the code in the next 12 weeks.
o Hopkins will go live in the spring, and the other sites will go live three months after Hopkins.
o Each site will have an observation period and a pre-observation period.
• Next Meeting to Discuss:
o Progress of BPA integration into EMR and pathway for data collection.
