Health Care Transitions Work Group Meeting Minutes

July 8, 2025

Attendees: Tim Bol, Amy Ohmer, Jennifer Iyengar, Kate Weaver, Kelli McWilliams, Len D’Avolio, Monica Gomberg, Jennifer Pfieffer, Tatum Sapper, Sara Lerner, Claire Rainey, Don Buckingham, Melanie Abrahamson-Sommer

• Blue Circle Health – An Introduction
• Creating a new measure: Adult Retention Metric

 Welcome and Introductions:

• Jenni Iyengar, MD, welcomed the group and made introductions.

Blue Circle Health

• Monica Gomberg, MD, introduced Blue Circle Health to the work group.
  • They are active in 10 States and expanding to more, many in regions where the Collaborative already has centers.
  • This is an adult only program that partners with your existing clinicians to extend additional care to people with Type 1 at no cost.
    • Peer support, education, insurance navigation, community, supportive counseling, and connection to resources are all offered.
    • This service is free regardless of insurance and immigration status.
  • Discussion occurred about rolling out to additional States, Helmsley Foundation involvement, and an EPIC add-on.

Measures

• Two options: create a new measure or work within the framework of a current measure. Ideally, we would like to work within the framework of a current measure.
• Discussion Questions: Can we define “transition” based on age? What age range?
• Can we get ED visits (endo on-call telephone encounters), as that information could be valuable? Is it possible to pull data for patients younger than 18?

Closing

• We need to decide how to move forward with this measure by dialing in the specificity and clarity of what we are trying to capture.
• We will be sending a Doodle Poll to the adult centers about a meeting in August; current available times are (All EST):
