  • READDY 2.0
  • Plan for HCT work group calls for 2025

Welcome and Introductions:

  • Faisal Malik, MD, announced a new Co-chair, Jenni Iyengar, MD.
    • Dr. Iyengar is an adult endocrinologist at the University of Michigan and wears many Quality Improvement hats.
  • To help Dr. Iyengar make all of the calls, and address a technical issue, we will be changing the format of this work group and sending out new Outlook invites. A Qualtrics survey will be sent out before the end of the month where you will be able to share your availability and have some agency over the next meetings schedules.

 Welcome and Introductions:

  • Faisal Malik, MD, announced a new Co-chair, Jenni Iyengar, MD.
    • Dr. Iyengar is an adult endocrinologist at the University of Michigan, spends most of her time seeing patients, but 40% of her time is dedicated to QI work.
  • In order to help Dr. Iyengar make all of the calls, and address a technical issue, we will be changing the format of this work group and sending out new Outlook invites. A Qualtrics survey will be sent out before the end of the month where you will be able to share your availability and have some agency over the next meetings schedules.

Survey Questions

  • Dr. Malik provided background information on the T1D Exchange Annual Survey Questions. The primary objective is to find out how many centers are meeting the six core elements of health care transitions for both adult and pediatric centers.
  • Discussion focused on Question #6, “Does your center provide tailored support for young adult patients (e.g., health care navigation education, peer support opportunities, increased emphasis on mental health) yes or no?
  • Dr. Iyengar suggested ways to meet of their young adults such as peer support, psychologists, care navigator, complex case manager or nurse manager, social workers, dieticians, and Dr. Malik suggested we create a scoring system to qualify how well a center is providing these services.
    • Discussion occurred about programs addressing specific to the needs of young adults, how to phrase that in a question, and how this question can extract what is needed, as well as defining that a young person is 18-26 years old.
  • The remaining discussion occurred about questions #7 and #8, “Do you have a system for tracking young adult patients from pediatric care entering your practice?” and “Does your center have a system in place for reaching out to someone who is lost to follow up?”
    • Discussion occurred about how to dovetail these two similar questions into one all-encompassing question, addressing patients lost to follow up.
    • Dr. Farahnaz Joarder, Oregon Health and Science University, mentioned a poster they are presenting at ADA next month which addresses this topic.

READDY 2.0:

  • Drs. Corathers and Malik have a new paper discussing READDY 2.0.
  • This update is more efficient and accurate with fewer questions.
    • A copy will be made available to the work group and if you would like free access to the READDY 2.0 tool, please fill out user agreement linked below.

Next Steps:

  • Dr. Malik reminded the group that our goal as a work group is to have clear measures that will be part of the data specs by November, for the Learning Session in Atlanta.
  • If your center is interested in sharing on a future call, please email Tim at tbol@t1dexchange.org or qi@t1dexchange.org to let us know.
  • New meeting times and invites – upcoming meeting invites will be deleted and replaced to address a technical issue and Tim will send out a Qualtrics survey to the Work Group to determine member availability and plan new meetings.
