  • Clinical center presentations
  • Children’s Health, UT Southwestern, Abha Choudhary, MD
    • CGM intervention: identified barriers to data sharing
    • Aim: sharing CGM data by 15%
    • Fishbone- no workflow for sharing data
    • Smart phrases added to Epic in Spanish and English
    • PDSAs: educated staff, smart phrases, website resources for patients, incentivize staff to add checkbox to clinic check in (food incentives)
    • Onboard patients to remote monitoring platform (with CGM sharing)
    • Baseline was 63%. Intervention period: 66%
    • Coaching pts to keep Dexcom app open, expand to Libre, expand to Plano campus, SDOH correlation
  • University of Utah, Intermountain Health, Allison Smego, MD and Vana Raman, MD
    • Addressing barriers to pump utilization and AID
    • Aim- improve glycemic outcomes and to increase pump use from 70-75%
    • Barriers in fishbone made intervention decisions: pump intro packets. Spanish pump class incorporated
    • Pump start checklist with flows
    • Offer quiz on knowledge gained after education. Nurses review and follow up
    • Tried to build acceptance of changed standards/processes from educators
    • Addressed mistrust about technology
    • 70% at baseline- increased to 84%. Increases gained for Hispanic and Black patients.
    • % of patients with A1c in goal range has increased.

    • Transitions intervention
    • Using Got Transition 6 core element steps
    • Aim- increase percentage being assessed using the READDY tool to 100%
    • Fishbone to ID barriers
    • Started with paper questionnaire
    • Made an Epic template documentation
    • 80% if cumulative patients of total eligible were screened with questionnaire
    • Some documentation/completion challenges
