Insulin Pump Transition after DKA

Question:

Does your center have a protocol on how to transition patients on insulin drip (DKA) back to their home insulin pump? i.e. How long do you wait after re-starting pump, before you turn off insulin drip and dextrose? Does your practice differ if the patient is on an AID system?

  1. Hi, Lily – Our Current DKA algorithm (https://www.uwhealth.org/cckm/cpg/diabetes-and-endocrinology/related/name-100682-en.html) has some wording related to this on page 2. We don’t transition directly from drip to pump very often (usually because families may not have all supplies with them), but if we do we would place a new site, have the family start the basal infusion on the pump, and stop the insulin drip and dextrose-containing fluids 30 min later. This is the same plan for when we administer glargine. This plan has existed for longer than I have been here and we are actively looking giving glargine earlier since we know that 30 minutes before stopping the drip is not best practice. We don’t spell anything out differently for an AID system.
