  1. Hi Whitney,
    We have developed a diabetes school management packet that CDCES completes in our EMR (i.e. Epic). It has drop down options for various treatment regimens (e.g., fixed dose, intensive insulin management, insulin pump). This can be done at in-person visits, telemedicine encounters, or upon caregiver request via phone. The Diabetes School Management Packet is a Letter format sent via MyChart to the family or can be printed off in clinic. We would be happy to share our template.

  2. We send an updated school care plan home with the patient at discharge from clinic appointments. We don’t necessarily send it with every visit, if no changes to insulin regimen have been made. Families are able to request a SCP and we can send to the patient on the patient portal (Cerner) in between visits. Hope this helps!
