Up-to-date School Care Plans

Question: How does your site handle patients on HCL insulin pumps switching to MDI and needing an up-to-date school care plan? Do your pump patients receive a pump school care plan and MDI care plan or are they directed to call the clinic if they have to come off the pump?

At TCH, we typically advise the family to reach out to us and we can send an updated school plan via MyChart.

We should probably do a better job of reminding families what to do if this happens. We do not include settings or MDI dosing on our pump school orders, so they would need to reach out to us. Sometimes a school nurse will do that if we haven’t provided orders and they need guidance. We have a lot of requests to list MDI doses on our pump plans (in case of failure) but we do not do that in current state. I’m interested in how you handle this.
