• Discussion on Creating a New Measure
• Transition Readiness Surveys
• Future Plan for HCT work group calls for 2025

 Welcome and Introductions:

• Faisal Malik, MD, welcomed the group and reviewed the agenda for the day.

Measures

• Sarah Corathers, MD, joined the conversation to review the measure around Transition Plan, as she had contributed to the first version of this measure.
  • Invite Anton and James, add some of these people from this work group to an upcoming agenda for the Data Science Committee.
• The measure was reviewed and can be located here: https://t1dx-qi.t1dexchange.org/data-mapping/
• Nicole reviewed the measure document and reported what would be required to get the recommendations added to the data spec.
  • The group would like to see measure development around improving transition readiness.
  • We will submit the measure as an agenda item at the next Data Science Committee meeting taking place on August 19^th, 2025.
  • This measure does not include people under 16 years of age.
• Discussion is ongoing about proceeding with refining the current measure or adding a new measure specific to transition readiness.
• Discussion occurred about how this measure will be used, and whether it should be general or specific, and next steps for measure development.
• Centers that are reporting: CMH, Cornell, NYU Langone, Helen DeVos, Seattle.

Closing

• We need to decide how to move forward with this measure by dialing in the specificity and clarity of what we are trying to capture and continue to do offline work through July, reconvene with draft language in August, while moving through the Data Science Committee process.
• Dr. Corathers reminded us of the challenges to consistent reporting.
  • Different centers have different resources, EHR workflows, IT support, and staffing models which is challenging across a Collaborative.

Next Steps:

• Adult Team meets in July.
• The Pediatric Only group meets again in August; we will come with draft language.
