    • If you don’t have a tool pick tool
  • Another way to think is increasing number of centers completing (x/6) core elements

 

Standardization on Assessment:

  • Consistently delivering Transition Assessment to all patients starting by age 16 annually

 

Next steps:

  • centers query baseline data of adolescents with T1D % age > 16 with an assessment within past 12 months
  • % T1D age > 18 with transition readiness assessment within past 12 months
