Two cases of stage 3 type 1, but monitoring caught both before DKA for rapid diagnosis and
treatment from IU and TCH.

  • Review of project aims and timelines
  • Monthly reporting
    • Challenges at Lurie: Texas offered to share information on reporting. Manual
      reporting is possible since the numbers are small. Dan will share flowsheet
      for Epic with Trevon. (Trevon will walk through data with Laura to support.)

    •  Texas Children’s
      • Webinar
      • New referral mechanism for PCPs
      • Edu for patients and families. Dev new onset binder on SIT Down T1D
      • Future: flyers in clinic and MyChart messaging
      • Dev flowchart to come to SIT Down T1D if they have a normal BMI
    • Children’s National
      • Cerner use. Working with informatics and using manual collection.
        Dev screening edu materials for patients who are diagnosed. Dev
        policies and procedures: screening guidelines, team deployment and
        roles, patient follow up process
    • Lurie
      • Tracking screening discussions and tracking in flow sheets for options
        for siblings
      • Increasing screening for pts with one other autoimmune condition
      • Educating providers on T1D screening for eligible populations. Plan to
        present at division meeting to increase screening outside of the dept.
    • Indiana
      • Increasing awareness of screening for family members on annual
        basis
      • Intentional with documentation, who qualifies, etc
      • Follow up on next visit. What are barriers?
      • EHR new templates in Cerner are being planned
      • Increase early-stage visits in referral pool for patients with prediabetes. Defining flow for who is appropriate for in person vs virtual
      • Increasing POC kits. Working with Trial Net team. Consent forms
        going to be added to REDCap. Explore education on capillary testing
  • KDD
  • PICK tool to identify what we should be doing immediately that are lowest effort
    • Create a referral link for screening
    • Telemed for screening
    • Edu materials
    • Flowsheets (high effort)
    • Enable kits are easy ($80-100 billing families) moving from research to
      clinical care. Carla is trying to get funders to support.
    • Will share PICK tool, templates.
  • Updates- will share PICK tool draft.
  • Next steps- start testing PDSAs. We will discuss in the next meeting. Share a week
    before the next meeting. We will share template.
