July Collaborative Call Meeting Minutes

  • Welcome & introductions, Nicole Rioles, MA Osagie Ebekozien, MD, MPH, CPHQ
  • Clinical center presentations
    • Barbara Davis Center, Emma Mason
      • Equity in tech access projects. Aim: increase Black and Hispanic pump access by 5%
      • Process map: identifying pain points. No pump start classes. ID that not everyone not using is asked/offered. Set to ID barriers through chart reviews
      • Asked pt: if they were asked before, if they used before, and why they weren’t. Interested. ID pts not using device and not meeting ADA A1c goals and had follow up scheduled
      • Implemented a questionnaire for people not using devices and not meeting A1c goals and asked about their barriers. 13% response rate (high visit cancellation rate)
      • Made pareto chart of questionnaire responses. Nervous about change and costs where the most common barriers. Offered education, alternatives and worked with pts, insurance, pharmacies, coupons to make adoption more affordable

      • Diabetes Distress project
      • They were not screening before Dec 2023
      • Started and expanded pilot project using 8 question screener for PwT1D. Goal: screen at least 70%
      • Fishbone: no process in place, high variation by site, lack of awareness among providers
      • Goal: automate and integrate into EMR
    • Icahn School of Medicine at Mt Sinai, Carol Levy, MD
      • Back up insulin for pump users (for emergency pump failures)
      • Goal: improve availability of back up basal insulin by 20%
      • Considerations: not always ask about backup, policies not in place, formularies change, need not always documented
      • PDSAs increase documentation
      • MAs screen and ask pts if they need backup scripts
      • Achieved goal 81% had appropriate codes and back up insulin documented
      • Mass Epic message to pump users as a reminder. Had improvement with strategy
      • Goal: automate strategy and make strategy active across the health care system to engage more care team members

      • T1D Exchange, in collaboration with the ADA, will be hosting a new diabetes equity focused conference titled, ADEPT. ADEPT 2024 will be held Tuesday and Wednesday, November 12-13 following the T1DX-QI Learning Session 2024 in Chicago, IL.
