Urgent, Weekend and Holiday Calls
Question:
For the pediatric centers: Currently at Cook the diabetes educators take any urgent call from Friday at 5pm-Monday at 8am and will also do inpatient education for newly diagnosed patients. I am trying to change the call portion and wanted to see if anyone had information I can benchmark from.Who is responsible for your weekend and holiday diabetes calls? Do the CDCES, fellow’s, endocrinologist, triage nurses (used for PCP or all specialties) take calls?
Additionally, how do you handle new onsets on weekends and holidays? Do the diabetes educators perform new onset education on the weekends and holidays? Thank you!

At Cleveland Clinic Childrens only the MDs are on call. We only have the support of our CDEs M-F during work hours and they provide our education for new onset T1D inpatient (we do not have resources to do outpatient unfortunately). Historically whichever MD who was on call would decide themselves if they felt comfortable doing the education component over the weekend to facilitate timely discharge. However we found that different teaching abilities, expectations etc were noted and that in fact patients who received the MD-education often had a more rocky start. For now we have decided that all of our new onsets will have CDE led new onset education since they have consistent skills and approaches.

Our providers take call after hours and on the weekends. We do have a telephone triage department at Rady and we are exploring building algorithms and protocols for the RN to use and then if they need the MD they would page the MD on call. Our medical unit RN’s are trained to do inpatient education if they are admitted and then we bring them to clinic after discharge to further their education. happy to share the protocols once they are built.
