Transitions of Care Working Group Meeting Minutes 9/12/2023

Deliverable:

• Examine and improve health care transitions practices
• Develop health care transitions toolkit to in cooperate best practices

Discussion:

• Jennifer Iyengar, easy to capture patients if coming directly from peds site, but hard to capture if outside the system, different type of diabetes can make it difficult as well
• Grace Nelson, No funding, good program but does not see all patients
• Lauren Waterman, catch area is 7 states, no formal system provider dependent due to catch area
• Meredith Wilkes, practice would alternate visits with adult endocrinology, trying to revamp program
• Manu, transition paper questionnaire to IPAD and upload to my chart
• Mark Clements, track survey from RED Cap into Cerner, transition coordinator, videos made to help with the transition discussion
• Vana Raman, transition coordinator, education visit before transition to adult

Next Steps:

• Centers to create process map to identify pain points in transition process
