Patient Engagement

I am wondering if there is a resource or any clinic site that may have a list of questions and/or a questionnaire that we can distribute to patients to get their input about how we can best help them to manage their diabetes. We received a DEI grant to apply towards helping to decrease the % of public pay patients with A1c’s greater than 9. We have purchased gift cards and medical center parking vouchers to offer patients who respond to some questions or a questionnaire. Now we just need to figure out how to best go about figuring out what questions to ask!
