Notes
Summary: Nicole shared patient developed article: Sustainable Generation of Patient-Led Resources in a Learning Health System
Development of toolkits to support the Collaborative and patient/family needs
Priority ideas generated by group:
- 1)Toolkit on outpatient care in medical/surgical as well as inpatient settings to support patient/family autonomy, device use, and good BG management during and after procedures
- What is being taught on the inpatient side?
- What do epidemiologists (and others outside of the endocrine space) understand about DM management?
- Shared decision making tools: Jeff recommended Atul Gawande’s book, Being Mortal
- 2)Jane Diabetes Update/ Grand Rounds: 6 fundamental things that we have learned about changes in diabetes care and management over the last decade (idea is that Jane could work this concept into a talk she is giving this fall and then share that talk as well at the October Collaborative call or November Learning Session)
- Concept: “Refresh” teaching/learning that is being passed down now to residents, fellows, and early to the new attendings
- 3)Emily C. and Jeff Develop/aggregate and share culturally appropriate resources that support diet/nutrition/exercise/language/lifestyle of multicultural patient populations

- 4)Craig Toolkit for tech and device upload/download to support more data-informed conversations during virtual and in person visits.
- 5)Katie Behavioral and Mental Health. Toolkit to reset and support PWD who are experiencing diabetes burnout. This excellent topic wasn’t mentioned during the meeting but it’s something that Katie emailed about later. She is specifically thinking about teen diabetes burnout and how we can support them.
Next steps and timeline:
- Patient/Parent Advisory (proposed) to present at October 26th Collaborative call
- Please take a look at the above list and let me know if you would like to be added to one of the 5 group concepts or if you would like to lead-co-lead one of them. I assigned people based on what I heard on the call yesterday- but we can add more folks on. We can also prioritize/timeline them so that we aren’t working on all at once.
- Next meeting will be scheduled for some time in week of October 11-15. Will send out Doodle poll to find best time/date.
Thank you all!
