Key Points:

Recap of Data spec review
- Data platforms: Smartsheets and EHR Mapped Data -> QI Portal/ Smartsheets
Discussion on phase 2 & 3 measures
- Distinguished what can be measured now vs what need to be built to measure in the near future
- Phase 2 measures
  - Time in range, standardized measure
  - Documented transition from peds to adult
    - Categorical (Yes/No if you screen and have documented plan, Yes/No referral)
  - SDOH screening
    - Hunger vital signs
    - Limitations if team does not have a social worker
    - These may change based on age groups
  - Phase 3 measures
    - Lost to follow up (no visit within the last 6 months, no future visit scheduled )
    - Review data patterns between visits
    - Adjust insulin does at least once since the last clinic visit (initiated by family or clinic)
    - Timing of insulin bolus (response of “At least several minutes before the meal” or “Immediately before the meal”)
    - Automated Insulin Delivery Use

Suggested modifications to existing data specs
- Field to flag clinics visits to endocrinology MDs/NPs/Other providers
- Binary CGM use field
